Grab the innocent one and pull them out of the way and protect them from the other erson but don't attack until there is no other option. Hit the attacker with an attack that doesn't cause too much damage and save the other person. Laugh and walk away or kill the innocent person yourself just to start a fight with the attacker. See no other option but to kill the attacker but feel real bad later. Send a small attack that only momentarily distracts the attacker, but the innocent person still gets hurt. Then walk away.

Causally head to the scene. Attack anyone, but don't kill. Kill the screamer without mercy. Melt into shadows and check the scene without being seen. Then decide whether to help or not. Rush over but hesitate before interfering. Rush to the area of the scream to help.
